Schedule Appointment

Orthopedic Specialty Associates accepts patients by referral. Appointment times are based on medical need and patient convenience. All minors under 18 years of age must be accompanied by a parent or legal guardian in order to be treated by our physicians. All new patients should arrive 30 minutes prior to their appointment time.

At the time of your appointment please bring:

• Photo ID
• Insurance cards
• New patient forms (if applicable)
• All diagnostic tests ( MRI, X-rays, CT scan)
• List of current medications
• Co-payments
